Are you a UK taxpayer?

If so, every £1 you give could be worth £1.25* to us, helping us fund our charitable work - at no extra cost to you.

Through the Gift Aid scheme, we can reclaim the tax on your donation or membership subscription. For every pound that you give we can claim an extra 25p from HM Revenue and Customs. We will also be able to reclaim the tax on any future donations you make.

I would like The Milverton Concert Society to reclaim tax on any eligible donations or membership subscriptions that I have made in the last four years and in the future, until further notice. I confirm that I pay the amount of UK income or capital gains tax at least equal to the tax that The Milverton Concert Society will reclaim.

* Charities can claim Gift Aid tax relief of 25p on every £1 that you give, making your donations worth a quarter more.

Please notify Milverton Concert Society via enquiries@milvertonconcerts.org if you:

  • Want to cancel this declaration
  • Change your name or home address
  • No longer pay sufficient tax on your income and/or capital gains.

If you pay Income Tax at the higher rate, you must include all your Gift Aid donations on your Self Assessment tax return if you want to receive the additional tax relief due to you.

You can pay for your membership either by BACS or by cheque.

Payment details will be contained in the acknowledgement email sent to you when you submit this form.
